We accept Medicare, Medicaid, and most Private Insurances for Primary Care. For patients with insurance, your copay will be due at the time of your visit and we will bill your insurance for any remaining balance.

Follow-Up Primary Care Visit
Billed to Insurance or $100 self-pay
Reviewing the lab results is a great place to start when it comes to optimizing your health.
Provider will help you interpret the information into an actionable plan, That is what will really make a difference in your health to help you live a healthier lifestyle.
Your need for new medications or follow-up labs/tests will be determined at this visit.
Appropriate Referrals to specialists will be sent as well if needed.
Telehealth Primary Care Visit
$50 Self-Pay
Please know that Telehealth is not appropriate for serious conditions that require immediate attention or emergency care. If you experience any life-threatening issues, call 911 or go to an emergency room immediately.
Visits are not billed to insurance and are self-pay.

The functional medicine model of care offers a patient-centered approach to chronic disease management. It seeks to answer the question, “Why are you ill despite my provider telling me all my labs are normal?”
We will gather your medical history and use this information to identify the root cause(s) of the illness, including triggers such as poor nutrition, stress, toxins, allergens, genetics, and your microbiome

Once we identify the triggers, we can customize a healthy living plan for you. Your plan will address many aspects of your life, from physical needs, including nutrition, exercise and sleep, to mental and emotional stressors related to social, work and community life.
